Step 1

Visit https://volunteerapp.lausd.net to submit an online application, or meet with Fariba Tavassoli (our Community Representative), at the school office for a paper application.

Step 2

Fill out the LAUSD Tuberculosis questionnaire form and submit with application.

Step 3

Use the form below to contact FOS and indicate your volunteer interests. Ms. Fariba will contact you once your application is approved.
